GLP-1s and Birth Control

Tirzepatide (Zepbound, Mounjaro) can quietly make oral contraceptives less reliable. Semaglutide doesn't — but pregnancy on any GLP-1 is a serious problem.

TL;DR: The FDA labels for Zepbound and Mounjaro warn that oral birth control pills may be less effective for 4 weeks after starting and after each dose increase. Switch to a non-oral method (IUD, implant, injection, ring, or patch) or use a backup method (condoms) during those windows. Semaglutide (Ozempic, Wegovy, Rybelsus) doesn't carry this warning.

Why tirzepatide is different

Tirzepatide slows gastric emptying more aggressively than semaglutide during dose escalation. Slower stomach emptying delays absorption of oral drugs, including the ethinyl estradiol and progestin in combination birth control pills. Studies showed a ~20% drop in contraceptive drug exposure at the first dose and after escalations. Semaglutide didn't produce a clinically meaningful reduction in the same trials.

Which contraceptives are safe on any GLP-1

  • Copper IUD (Paragard)
  • Hormonal IUD (Mirena, Kyleena, Liletta, Skyla)
  • Contraceptive implant (Nexplanon)
  • Depo-Provera injection
  • NuvaRing (vaginal ring)
  • Xulane, Twirla (transdermal patch)

All of these bypass the stomach, so slowed gastric emptying doesn't affect them.

If you're staying on the pill

Use backup contraception (condoms) for 4 weeks after starting Zepbound or Mounjaro, and for 4 weeks after each dose escalation. That's the FDA-recommended window. Take your pill at the same time daily — missed doses combined with slower absorption compound the risk.

Pregnancy on a GLP-1

All GLP-1s are pregnancy category X or its modern equivalent — animal studies show fetal harm, and there's no adequate human data. Stop the drug immediately if you become pregnant or plan to. Most guidelines recommend discontinuing GLP-1s at least 8 weeks before trying to conceive.

Emergency contraception

Plan B (levonorgestrel) is an oral pill and may be affected by slowed gastric emptying on tirzepatide. Copper IUD as emergency contraception (placed within 5 days) is more reliable in that scenario and worth asking about.

Talk to your prescriber

Bring this up at your intake or at your first dose escalation. Most telehealth clinicians will proactively recommend switching to a non-oral method for tirzepatide patients; if yours doesn't, ask.

FAQ

Does Zepbound affect birth control pills?

Yes. The FDA label warns that tirzepatide (Zepbound and Mounjaro) can reduce oral contraceptive absorption, especially during dose escalation and for 4 weeks after each increase.

Does Wegovy or Ozempic affect birth control?

Semaglutide has a much smaller effect and no FDA label warning for reduced contraceptive efficacy. It slows stomach emptying but hasn't shown the same absorption issue.

What contraceptives are safe on tirzepatide?

Non-oral methods: IUD (copper or hormonal), implant (Nexplanon), depot injection, ring, or patch. These bypass the stomach and aren't affected.

How long is the risk period?

The 4 weeks after starting Zepbound/Mounjaro and the 4 weeks after each dose increase.

Can I get pregnant on a GLP-1?

Yes — and you shouldn't. GLP-1s are contraindicated in pregnancy. Effective contraception is required.
